1. Recognize and understand that martial arts training is an activity that involves physical contact and that my participation might result in
serious injury, including permanent disability or even death, and severe social and economic loss.
2. Recognize and understand that such risk may be due to not only my own actions, but also the action, inaction or negligence of others, the
regulations of participation, or the conditions of the premises, or of any of the equipment used.
3. Recognize that there may be other risks that are not known to me or to others or not reasonably foreseeable at this time.
4. Agree to inspect the facilities, equipment and pairings prior to participation. I will immediately inform an instructor if I believe that
anything is unsafe or beyond my capability and refuse to participate.
5. Assume all of the foregoing risks and accept personal responsibility for any damages that may result from injury, permanent disability or
death.
6. Enter martial arts training and/or competition entirely of my own free will and understand the importance of following the rules of
training and competition.
7. I certify that I am in good physical condition, and have no disease, injury or other condition that would impair my performance or physical
and mental well-being during intense training practice and/or competition.
8. Grant permission in case of injury to have a doctor, nurse, athletic training or other emergency medical personnel provide me with
medical assistance or treatment for such injury.
9. Release, waive, discharge and covenant not to sue, Kokoro Shotokan Karate-Do, its affiliated organizations and governing
bodies, their officers, instructors and personnel, other members of the organizations, participants, supervisors, coaches, sponsoring
organizations or their agents, and if applicable, owners and leasers of the premises from any and all liability to the undersigned, his or her
heirs and next of kin for any and all claims, demands, losses and damages which may be sustained and suffered on account of injury,
including death or damage to property, caused or alleged to be caused in whole or in part by the negligence of the realeasees or
otherwise.
10. I grant Kokoro Shotokan Karate-Do permission to take indirect and/or direct photos and videos for all karate related posts, social media, flyers, advertisements and record keeping of me and or my child.

Dojo Kun
Karate is more than physical training—it is a path for developing strong character and discipline.
The Dojo Kun reminds us of the values we practice in the dojo and in everyday life:
- Seek perfection of character
- Be faithful
- Endeavor
- Respect others
- Refrain from violent behavior
“Do” means “the way.” Through karate training, students develop both mental and physical stamina. Students are encouraged to persevere and never give up. Without earnest effort and perseverance, the true spirit of karate cannot be understood.
Dojo Etiquette
- Bow when entering and exiting the dojo; ensure you’re bowing facing towards the dojo. Bow to staff when entering and exiting and bow prior to stepping on the mat. Bow properly during Mokusou and thank and bow to your instructor at the end of each class. In addition, thank your classmates whom you trained with.
- When you hear “Line up” or “One line,” stand shoulder to shoulder facing the front of the dojo in rank order.
- When bowing in during “Mokusou,” practice quiet meditation by sitting up straight with good posture, closing your eyes, and emptying your mind.
- During class proper respect must be maintained at all times. If Sensei asks a student to come to the front to demonstrate, the student should say “OSS” and run to the front of the class. Bow to Sensei before demonstrating and bow again when instructed to return to the line, saying “OSS”.
- Students should always show their instructor they understand and respect instruction by replying with “OSS” loud, clear, and short.
- Never boast about your rank or skill.
- Do not teach other students unless instructed to do so by Sensei.
- Respect the dojo including cleaning up after yourself and respecting the bathroom, lobby, chairs, and all dojo equipment.
- Do not speak unless spoken to during instruction.
- Zero bullying is allowed in the dojo.
- Arrive on time with your gi and belt neatly on and ready to train.
- If you show-up to class, see it through. Do not leave class early.
Students Policy
- The dojo is a formal place where all participants must wear a gi (a plain white karate uniform). Wash your gi and belt regularly and keep it in good condition.
- Fingernails and toenails must be cut and kept short before class.
- Wear dojo style slippers when off the mat (crocs, sandals, etc.).
- No jewelry or watches of any kind are permitted during training.
- Place your bags and water bottles in the cubbies provided.
- Remove your shoes and place them neatly along the mat prior to stepping on the mat.
- No unnecessary talking during practice.
- Never leave the mat without permission from the instructor.
- No eating or drinking including gum on the mat.
- If you need to leave class early, let the instructor know before class begins.
- Clean your gear after each use with disinfectant.
- Shin and boot protectors, gloves, and gi are mandatory gear for training. Only approved karate style gear is permitted.
- Follow dojo Kun and etiquette at all times.
- Make every effort to attend all Seminars at the dojo. Follow all instructions provided and etiquette for each seminar.
Spectators and Parents Policy
- No food or drinks other than water are permitted inside the dojo. No open cups or cans are allowed.
- Noise should be kept minimal so students and instructors can focus during class. Please keep voices low by whispering and take young children outside if they become too loud during class.
- All spectators should stand during bow in (Mokusou). Bowing toward the Sensei, your children, and other students is a sign of mutual respect. We are one in the dojo.
- Please arrive on time with your child fully prepared for class.
- Pay dues on time and address any missed payments within 3 days to avoid a late fee. After 3 days your child will not be permitted to attend classes until the balance is paid in full.
- Monthly dues are charged automatically with a credit card on file. If your card declines, you must make the payment within 3 days to avoid a $15 late fee for each child account that is past due.
- We do not accept drop-in fees. Students must have a full membership to attend classes.
- You permit Kokoro Shotokan Karate-Do and its employees to record and take pictures of you, your family members, and your child for dojo related content that may be used for Instagram, Google, other social media, flyers, and other dojo related printing. You have the right to opt out. Please send a written request to kokoroshotokankarate@gmail.com.
- Refer a Friend credit may be used for non-family members (anyone outside of the home). A $25 credit is applied to your child’s monthly membership once the new referred member signs up for a monthly plan. The credit may also be used toward dojo gear. For charter school students whose memberships are paid by the school, the credit may be applied toward summer membership when charter funds are not applied. All credits must be used within 3 months from the date they were awarded unless applied to a summer membership within the same year.
- Gear such as shin and boot protectors, gloves, and gi are mandatory. Only approved karate style gear is permitted.
